Services Provided
Ground Rules Academy agrees to provide martial arts instruction and training to the Student(s) in accordance with this Agreement.

1. Term
   This Agreement begins on the date of submission and continues on a month-to-month basis until terminated by either party or superseded by a pre-paid promotional package.

2. Monthly Dues Options: The Student or Parent/Guardian agrees to pay monthly dues for each Student according to the selected membership option. Monthly dues are due on the 15th of each month. The first payment—prorated from the enrollment date to the 15th of the following month—is due upon signing this Agreement.

   Available monthly options include:

   • Standard Membership: $175.00 per month. This provides access to all regularly scheduled classes.

   • Women-Only Class Membership: $75.00 per month. This provides access exclusively to the weekly women-only class and open mat sessions.

   A 10% discount (reducing dues to $157.50 per month for Standard Membership or $67.50 per month for Women-Only Class Membership) is available for the following:

   • College students (with valid student ID).

   • Family plans (two or more immediate family members enrolled simultaneously in the same membership type).

   • First responders (e.g., police, firefighters, paramedics with valid ID).

   • Active duty military or veterans (with valid military ID or proof of service).

   Eligibility for discounts must be verified at the time of signing, and Ground Rules Academy reserves the right to request supporting documentation. Discounts apply only to the applicable monthly rate and are not combinable unless otherwise specified.

   Pre-Paid Package Option: Alternatively, the Student or Parent/Guardian may elect a pre-paid package for each Student. These packages are available only during designated promotional periods each year and require full payment at the time of purchase:

   • 12-Month Package: Available for purchase from August 15 to September 15 and from December 15 to January 15 each year. This package covers 12 months of access under the selected membership type (Standard or Women-Only Class) from the date of purchase or activation.

   • 6-Month Package: Available for purchase from August 15 to September 15 and from December 15 to January 15 each year. This package covers 6 months of access under the selected membership type (Standard or Women-Only Class) from the date of purchase or activation.

   No monthly dues will be collected during the term of a pre-paid package. Pre-paid packages are non-refunded, and no refunds will be provided for any portion of the payment, even in the event of early termination. Upon expiration of a pre-paid package, the Agreement reverts to the monthly dues option unless a new package is purchased or the Agreement is terminated.

3. Payment Method
   Payments for monthly dues must be made via debit or credit card. The Student or Parent/Guardian agrees to maintain valid payment information for the duration of this Agreement. Pre-paid packages may be purchased with cash, check, venmo, or zelle.

4. Late Payments
   A $25.00 late fee will be applied if payment for monthly dues is not received within 10 days of the due date. If the account remains unpaid for 30 days, Ground Rules Academy may suspend services until the account is current. This does not apply to pre-paid packages, as full payment is required at signing.

5. Termination
   Either party may terminate this Agreement with at least 48 hours’ notice (written or verbal) before the next billing date for monthly dues or the end of a pre-paid package term. The Student or Parent/Guardian is responsible for all dues and fees incurred up to the termination date.

6. Refunds
   No refunds will be provided for any payments, including monthly dues or pre-paid promotional packages, upon termination.

7. Student Responsibilities
   The Student(s) agrees to:

   • Follow Ground Rules Academy’s etiquette and expectations, including safety, appropriate clothing, hygiene, and conduct.

   • Treat instructors and fellow students with respect.

   • Exercise ordinary care while using the Facility to protect themselves and others.

8. Ground Rules Academy Responsibilities
   Ground Rules Academy agrees to:

   • Maintain a safe and clean training environment.

   • Offer a variety of classes to meet student needs.

9. Liability Waiver
   The Student(s) and Parent/Guardian acknowledges that martial arts training involves physical activity and accepts all associated risks. The Student(s) and Parent/Guardian releases and holds harmless Ground Rules Academy, its instructors, and staff from liability for any injuries or damages during training or on the premises.

10. Location
    If the Facility is unavailable due to fire, act of God, catastrophe, accident, lease expiry, or any other reason, the Student(s) shall use the nearest alternate facility provided by Ground Rules Academy.

11. Lost or Stolen Property
    Ground Rules Academy is not liable for any lost or stolen property of the Student(s) or their guests.

12. Training Schedule
    Training sessions follow the posted schedule. The Student(s) acknowledges that sessions may be cancelled due to instructor absence caused by illness, injury, or travel delays.

13. Entire Agreement
    This Agreement constitutes the entire agreement between the parties, superseding all prior understandings, whether written or oral.

14. Governing Law
    This Agreement is governed by the laws of the State of California.

15. Severability
    If any provision is invalid or unenforceable, the remaining provisions remain in full force.

16. Indulgences
    No failure or delay by Ground Rules Academy in enforcing any right under this Agreement constitutes a waiver. Amendments or waivers must be in writing, except as provided herein.

Electronic Signature

By checking the box below and typing my name, I acknowledge that I have read, understood, and agree to be bound by the terms of this Agreement. I consent to conduct this transaction electronically, and my typed name serves as my electronic signature, equivalent to a handwritten signature under California law.

Note: For minors under 18, a parent or guardian must also sign.
